Category images are missing. Tried fresh install and re-install. The images are still missing.

No difference.

This may means only one thing. Your images folder has no write access. Please fix your file permissions. I checked it again and it does install files. Or check if you have

joomla root/images/forum/

folder with files.

Also check path to image in the source code. May be something wrong there.

Messed up MAMP folder permissions.
