To work properly with all conditions, this line needs to be included at the top of the template code:

$value = array() ;

When Title display = NO , it still shows the title header.

2 Questions to the new "First position width":

1) Would it not be better to just add the width to the header instead of replicating it in every row?

2) Why is the width attribute used twice per cell - as attribute and as style?

What if header is tuned off?

Because some templates apply styles to table and simple attribute not always work properly. Just in case to ensure, I have added style too.
