Just wait till next update tomorrow. There will be new parameter for form title in form template parameters.

Hi there, I have done the upgrade but cannot seem to find this. I assume there should be a place where I can manually input the field but I can't locate one on the submission form template.

Judah,

if you only modified the template because of the title, i would suggest to delete your template and create a new one as a copy of the "default". Then you will find the missing parameter there.

Otherwise it depends on the magnitude of changes. If it is a heavily modified template, the solution would be to apply the new changes to your template and as suggested by Maik Kaune, also add the new parameter to the corresponding XML file.
